NEW CLIENT REGISTRATION

Congratulations on your decision to create a healthier lifestyle and thank you for choosing Body One Wellness to help! Below you will find links to required forms and assessment materials. These include Consent and Physical Activity Readiness Forms and a General Medical History Questionnaire. These forms need to be reviewed, completed, signed and provided to your trainer at your scheduled assessment. To learn more about your overall health, please also track what you eat for at least 4-5 days (at least 1 day being a Sat/Sun). You can use the meal tracker template below or simply write it in a notebook following this format. Please pay particular attention to portion sizes and note them accordingly. Please also record the times you eat as well. The nutrient grams and calorie columns are here, so if you know them, please keep track of those as well.

THE ASSESSMENT, YOUR FIRST SESSION
The first session will include an overall assessment in which includes BMI reading, Circumference Measurements, Nutrition Analysis, Cardio Analysis, Movement Screening and Goal Setting. The assessment is used to determine your current health and fitness level and provides the detail we need to create your plan to help you achieve your personal goals. Please plan for about an hour, come dressed for movement and measurements with skin fold calipers. Sports bras work well for women. If you have a heart rate monitor please bring that along as well. We look forward to working with you and learning more about your personal health and wellness goals.
